Informed Consent

Our Informed Consent serves to provide information to our clients about the therapy process. By completing this form, clients authorize treatment and agree to specific mental health interventions. 

​Telehealth Informed Consent 

Our Telehealth Informed Consent provides further information to our clients about conducting therapy sessions remotely through telecommunication either by video or phone. By completing this form, clients authorize treatment and agree to specific mental health interventions through HIPAA compliant platforms. 

Privacy & Practices Notification ​

Our Privacy and Practices Notification details rights and responsibilities about how our client's personal health information is protected, used and disclosed.

Release of Information

Our Release of Information is required to release information to any outside agency, entity or individual. Your permission  is necessary before your health records are shared for any other reason. Exceptions are outlined in our Privacy and Practices Notification.
